This position has the flexibility to be 100% remote. 

As a PCA Program Navigator you will assist families, counties and other agencies regarding a client’s services.  As a Navigator you will be responsible for a variety of client related tasks within the PCA Program.

Requirements: 

  • 1-year experience working with individuals having developmental disabilities; 2 year degree preferred but not required.
  • Experience working with Waivered Services and/or PCA is a plus
  • Maintain excellent communication with families, counties and state agencies
  • Advanced communication skills both written and oral
  • Ability to organize and prioritize
  • Ability to handle numerous tasks simultaneously while maintaining strong attention to detail
  • Maintain a high standard of professional courtesy, flexibility, confidentiality while working in a fast-paced office environment, independently and as a team member.
  • Advanced skills in the use of office technology, including MS Office Suite with advanced skills in Word Excel and Outlook
  • Valid driver’s license.

Essential Functions:

  • Assist families, counties and other agencies who call our office to navigate their PCA services
  • Schedule intake home visits with families
  • Communicate with counties, families and health programs daily in an effort to coordinate services
  • Review Service Agreements for accuracy with start dates
  • Maintain program required documentation
  • Coordinate service delivery and accommodation with other members of the Accra team, including Qualified Professionals, HR staff and others.
  • The ability to explain sometimes difficult concepts verbally and in writing to customers and coworkers.
  • Ability to maintain a calm and professional demeanor when problem solving with occasionally demanding customers and coworkers.
  • Ability to maintain a dependable work attendance and engage as part of a collaborative and integrated team

This position is remote, but does require traveling to clients homes in the surrounding Metro counties. 

The IHS Designated Coordinator is responsible for meeting with, setting up, and proving ongoing training and support to individuals accessing Accra’s Individualized Home Supports with Training (IHS) program (formerly ILS).

Essential Functions:

  • Meet with clients and representatives to complete required paperwork, including, but not limited to, initial Intake/Annual Paperwork, Clinical Notes, Adult Protection Reports, Client Plans, and Client Discharges.
  • Ongoing client caseload management including establishing professional therapeutic relationships, diagnosis knowledge of assigned clients, maintenance of client files, and, when needed, providing client transportation according to established transportation policies.
  • Communicate with assigned Client Representatives, IHS Manager, and/or IHS Supervisor of important client issues or concerns, including ongoing assessment of client goals and suggested changes to said client goals.
  • Monitor each client’s authorized hours, ensuring client interactions stay within the allotted number of authorized hours and follow procedures for any needed changes to client hours.
  • Prepare for, and attend, monthly supervisory meeting and/or periodic client plan reviews.
  • Complete required trainings, as assigned.
  • Provide services in a person-centered manner that aligns with the Mission and Vision of Accra.
  • Work with leadership and other staff on the development of new processes and procedures.
  • Maintain knowledge and understanding of DHS Home and Community Based Service Waivers, 245D Statutes, and best practices regarding service delivery.

Requirements: 

  • Associate or Bachelor degree in Human Services; preferably Occupational Therapy or Social Work.
  • 1 year experience in a Human Service field.
  • Demonstrated creativity, critical thinking, and problem-solving skills.
  • Excellent organizational, problem solving, communication, and interpersonal skills.
  • Ability to travel independently across the Accra geographic service areas.

The Outreach Specialist will be responsible for outbound communication efforts to education and resolve client issues that may cause an interruption in their homecare services. Your primary focus will be to enable clients and responsible parties (RPs) to be able to proactively safeguard their homecare services.

Requirements:

  • One year of experience working with clients over the phone, but inbound and outbound.
  • Experience working with waivered services or PCA homecare services would be nice to have.
  • Self-starter with the ability to take initiative and problem solve.
